Soccer Manager believes that most of the answers to your questions are covered within our Help.

Answer

It depends on your tactics indeed. If eg. your midfielder is injured at #85 and you have done only 1-2 sub's or none then normally SM will replace him for u, but if sub's completed then you go down with 10 players. So you must always have at least one player-substitution for each position, better for my opinion: GK, CB, RB, LB, FW, CM(or DM), Wing.
